Arriving to your appointment:

When you arrive for your appointment, please have the following with you:

  • Photo ID
  • Insurance card or insurers information
  • List of current medications
  • Summary of your health concerns

Cancelling an appointment:

If you cannop keep your appointment, plese give us at least a 24 hour notice. There will be a $25 charge for cancellations within 24 hours of your appointment and No-Shows. Additionally, if you will be unavoidably late for your appointment, please call us to let us know. Please note that if you are more than 10 minutes late, you may be asked to reschedule.

Insurance:

We are participating providers for most major health insurance companies. Please contact your insurance company to find out if our providers are on your specific plan and if you have any plan restrictions or requirements. Co-pays are due at each visit and will be collected when you check in. For your convenience we accept payments by cash, check, Visa and MasterCard. We do not accept Medicaid or Medicare.

Copying Medical Records:

Your medical records can be copied and released after we receive a signed request. There is a copying fee that is due at the time of release of the records. You will be informed of these charges. Please allow a minimum of 72 hours to process your copy request.

Prescription Refills:

The most effective and accurate way to refill an existing prescription is to call your pharmacy. The pharmacy will fax a refill request to our office. Requests will be processed during regular business hours Monday-Friday.

If this is a new prescription that our office has not prescribed for you before or you have not been seen in our office within the past year, you will need to call the office and speak with a medical assistant.

Completion of Forms:

Outside agencies may request forms to be completed by their provider (e.g. disability, FMLA, return to work form, etc.). It is impossible to complete these requests immediately. For this reason we require patients to fill in as much information as possible before submitting the forms to us. Please allow a minimum of 72 hours for forms to be completed.

Patient Confidentiality:

Your health information is protected and patient confidentiality laws limit whom we can release information to. We will only disclose your information when you have given permission by a signed request or listed a representative on your account. Please complete the HIPPA Release for above if you’d like a representative to have access to your health information.

Hospitals / Surgical Facilities:

Our physicians have privileges at Utah Valley Regional Medical Center, Timpanogos Regional Hospital and Central Utah Surgical Center. To learn more about their services or billing, please contact the facilities directly.

Office Courtesy:

We ask you to inform the receptionist at the time of scheduling your appointment if you are experiencing flu-like symptoms. If you do arrive at a visit sick, the receptionist may ask you to wear a mask to protect other patients. For the safety of our pregnant patients, we request sick children be left at home. Please consider that children can still be contagious for up to seven days after their symptoms have passed.

Please refrain from bringing food or beverages to the office. We would like to be respectful of all of our patients, especially of those who may have a sensitive stomach. If you do bring something in, the receptionist may ask you to eat outside the office suite, in the common space of the building.

After Hours / Weekend Instructions:

The office is available Monday-Thursday 8am-5pm, and Friday 8a-3pm. If you need medical assistance after these hours or during the weekend, please call the office and you will be given instructions to reach the oncall physician. If you have business related questions or need to schedule an appointment, we ask you call back during regular office hours.

A physician will always be available to respond to emergencies. If it is after hours and you think you are in labor, we ask you to go to Labor and Delivery where the nurses will immediately evaluate you and call your physician directly.

Insurance Guide

We are committed to being as accessible to our patients as possible. This is why we accept most health insurance programs. If you have medical insurance that covers the anticipated care, please bring your insurance card with you to your visit. If you do not have a copy of your insurance card, please be prepared to provide complete information of the insurance company name, address and policy number. In order to submit your insurance claim, we require accurate and complete information to do so.

Please check with your Insurance provider prior to your appointment. There may be specific requirements or restrictions with regards to which hospital, laboratory or services, etc may be covered within your plan. Although we are familiar with most guidelines, it is the patient’s responsibility to inform the office of these requirements or requests.

If your services are not covered by an insurance health plan or we cannot verify coverage, we will expect payment of office services at the time of service.
